CEPHAZOLIN/CEFAZOLIN

Indications & Dose:

 Infections of the lower respiratory tract, urinary tract, biliary tract, and genitourinary tract; infections of the skin and skin structure, bones and joints; endocayditis, prostatitis, surgical prophylaxis, septicemia


DOSE


Adult


Usually 1-4g daily i.m. or i.v. in 2-3 divided doses.


• Life-threatening infections: IM/IV 1-2 g q6h


• Mild/moderate infections: IM/IV 500 mg-1gm q8h


• UTI: 500mg-1gm 12 hourly


• Surgical prophylaxis: IM/IV 1 g 1/2-1 hr prior to surgery then 500 mg-1 g q6-8 hr x 24 hr postoperatively


Dosage reduction appropriate in renal impairment (CrCI <50 ml/min)


Child >l mo


• Life-threatening infections: IM/IV 100 mg/kg in 3-4 equal doses


• Mild/moderate infections: IM/IV 25-50 mg/kg in


3-4 equal doses


Below 1 month not recommended

Contraindications:

 Infants < 1 mo


 

Side Effects:

CNS: Chills, dizziness, fever, headache, paresthesia, weakness


GI: Abdominal pain, anorexia, bleeding; diarrhea, glossitis, increased LFTs, nausea, vomiting, pseudomembranous colitis


GU: Candidiasis, nephrotoxicity, pruritus, vaginitis


HEME: Bone marrow suppression, eosinophilia; hemolytic anemia, lymphocytosis


SKIN: Dermatitis, rash, urticaria

Hypersensitivity to penicillins, renal disease

Aminoglycosides: Additive nephrotoxicity


Ethanol: Disulflozm-like reaction


Chloramphenicol: Inhibits antibacterial activity of cefazolin


Loop diuretics: Increased nephrotoxicity


Oral Anticoagulants: Additive hypoprothrombinemia
