Clubbing of fingers, retinal haemorrhages, sple-nomegaly (subacute), splinter haemorrhages, tachycardia, Janeway lesion

Dd:

Rheumatic fever, SLE, periarteritis nodosa, syph-ilis, lymphoma, brucellosis, TB, cardiac tumour or aneurysm, aortic aneurysm

Phys:

Commonly occurs in previously damaged or ab­normal hearts, particularly after rheumatic fever. Usually involves mitral or aortic valve. Insidious in onset with fever, petechiae, splenomegaly, anae­mia and cardiac murmur
