Febuxostat

Indications & Dose:

Chronic hyperuricaemia where urate deposition has occurred. Febuxostat is recommended as an option for chronic hyperuricaemia in gout when allopurinol is contraindeicated or not tolerated.

Adults: 80mg once daily, increasing to 120mg once daily if serum uric acid >357 micromoles per litre after 2-4 weeks.

Children: Not recommended.

Prophylaxis and treatment of acute hyperuricaemia with initial chemotherapy for haematologic malignancies

Adult: 120mgoncedaily,tobestarted 2 days before startofcytotoxictherapyandcontinuedfor7–9days, accordingtochemotherapyduration 

Contraindications:

Acute gout, ischaemic heart disease, congestive heart failure, organ transplant recipients, conditions involding increased urate formation, pregnancy, lactation

Thyroid dysfunction. When initiating theraphy also give colchicine or NSAID for6 months. Perform LFTs, before and periodically during treatment. 

Interaction:

Mercaptopurine, azathioprine, theophylline, glucuronidation inducers.
