Deferiprone

Indications & Dose:

Treatment of iron overload in patients with thalasemia major for whom deferoxamine therapy as contraindicated or who present serious toxicity with deferoxamine therapy


DOSAGE  :


25mg/kg body weight/ three times a day, max;


100mg/kg/day. 

Contraindications:

Neutropenia. agranulocytosis, pregnancy and lactation

Precautions:

Monitor neutrophil count, serum ferritin & Zn conc regularly. Discontinue if infection develops. Renal & hepatic impairment. Children.

DRUGS


antacids or mineral supplements containing polyvalent cations such as aluminum, iron, or zinc:


Decreased effectiveness of drugs containing polyvalent cations drugs known to cause agranulocytosis orneutropenia: Additive adverse hematological effects


UGT1A6 inhibitor such as silymarin (milk thistle): Possible increased risk of deferiprone-related adverse effects


 

Warnings:

Adverse Effects:

CNS: Cerebellar syndrome, cerebral hemorrhage, chills, depression, enteroviral encephalitis, gait disturbance, headache, impaired psychomotor skills increased intracranial pressure, obsessivecompulsive disorder, pyramidal tract syndrome, pyrexia, seizures, somnolence


CV: Atrial fibrillation, cardiac failure, hypertension, hypotension, peripheral edema


EENT: Diplopia, epistaxis, hemoptysis, papilledema, parotid gland enlargement, pharyngitis, periorbital edema, retinal toxicity, teeth grinding


GI: Abdominal pain, change in appetite, diarrhea, dyspepsia, elevated alanine ami-notransferase levels, enterocolitis, gastric ulcer, hepatomegaly, infectious hepatitis, jaundice, nausea, pancreatitis, rectal hemorrhage, vomiting


GU: Chromaturia, elevated BUN levels, glycosuria, hemoglobinuria, increased serum creatinine levels


HEME: Agranulocytosis, neutropenia, pancytopenia, thrombocytosis


MS: Arthralgia, arthropathy, back or extremity pain, chondropathy, difficulty opening jaw, myositis


RESP: Acute respiratory distress syndrome, pneumonia, pulmonary embolism


SKIN: Cryptococcal cutaneous infection, HenochSchonleim puepue, hyperhidrosis, photosensitivity, pruritis, pustular rash, rash, subcutaneous abscess


OTHER: Anaphylactic shock, dehydration, hypersensitivity, metabolic acidosis, multi-organ failure, sepsis, weight gain
