Hydroxyprogesterone

Indications & Dose:

 Amenorrhea (primary and secondary), dysfunctional uterine bleeding, metrorrhagia, metastatic endometrial carcinoma (palliative therapy). Threatened abortion, habitual abortion.


DOSAGE


Adult


Usual dose 250-500mg weekly during 1st half of the pregnancy.


Amenorrhea and uterine bleeding: IM 375 mg, repeated q4 wk prn


Endometrial carcinoma: IM 1-7 g/wk


Habitual abortion: After confirmation of pregnancy, 250-500mg IM at weekly intervals.


Threatened abortion: 500mg IM 2-3 times weekly.

Contraindications:

Active thromboembolic disorders or thrombophlebitis, cerebral hemorrhage, impaired liver function or disease, breast cancer, undiagnosed vaginal bleeding, missed abortion, use as a diagnostic test for pregnancy

Side Effects:

CNS: Depression, dizziness, fatigue, headache


CV: Edema


GI: Anorexia, cholestatic jaundice, increased weight, nausea, vomiting


GU: Amenorrhea, breakthrough bleeding, breast changes, dysmenorrhea, gynecomastia


METAB: Hyperglycemia 


SKIN: Acne, alopecia, hirsutism, melasma, rash

Epilepsy, migraine, asthma, cardiac or renal dysfunction, depression, diabetes

Aminoglutethimide: Decreases progestin concentration

 Pregnancy category D: an increased risk of hypospasdias in the male fetus and mild virilization of the female fetus have been reported with progestin use; progestins compatible with breast feeding
