Indications & Dose:
Temporary maintenance of potency of the ductus arteriosus until definitive surgery can be performed, erectile dysfunction, primary pulmonary hypertension, severe peripheral arterial occlusive disease
DOSE 2.5 microgram intracavernous inj if necessary 2.5 increment, max 60 mg for 1-3 weeks
Contraindications:
Conditions that might predispose to priapism (e.g., sickle-cell anemia/trait, multiple myeloma, leukemia), patients with anatomical deformation of penis (e.g., Peyronie’s disease), penile implants, far sexual intercourse with a pregnant woman ( unless a condom is used)
Side Effects:
CNS: Cerebral bleeding, fever, hyperextensian of the neck, hyperirritability, hypothermia, jitteriness, lethargy, seizures,stiffness
CV: Bradycardia, CHF,edema, flushing, hypotension (4%), tachycardia, ventricular fibrillation
GI: Diarrhea, hyperbilirubinemia, regurgitation
GU: (Penile administration): Injection site hematoma, penile fibrosis, (3-8%), penile pain, (36%), prolonged erection
HEME: Bleeding, DIC, thrombocytopenia
METAB: Hyperkalemia, hypoglycemia, hypokalemia
MS: Cortical proliferation of the long bones
RESP: Apnea (10% of neonates), bradypnea, hypercapnia, tachypnea, wheezing
