STAVUDINE

Indications & Dose:

Treatment of HIV-1 infection in combination with other antiretroviral agents 

• BY MOUTH

Adult
• PO: under 60 kg 30mg every 12 hour , perferably atleast one hpur before food , 60kg and over 40mg every 12 hour.

Neonates: under 2 weeks 50microgram /kg every 12 hour

Child
• PO: If weight 30 kg, give recommended adult dose; if weight < 30 kg, 1 mg/kg/dose q12h
•Renal insufficiency: CrCl 26-50 ml/min, 20 mg q 12h if weight
• 60 kg, 15 mg q 12h if weight <60 kg; CrCl 10-25 ml/min, 20 mg q24h if weight
• 60 kg, 15 mg q24h if weight
<60 kg; hemodialysis: 20 mg q24h (< 60 kg or 15 mg q24h (< 60 kg) given after hemodialysis and at same time of day on nondialysis days 

Contraindications:

Side Effects:

CNS: Dementia, headache, insomnia, peripheral neuropathy (15%-21%)

GI: Abdominal pain, diarrhea, increased transminase, lactic acidosis, nausea, pancreatitis

HEME: Neutropenia

MS: Myalgia

SKIN: Rash

MISC: Asthenia, fever, lactic acidosis 

Cautions:

Precautions:

Peripheral neuropathy, pancreatitis, renal insufficiency, folate or vitamin B12 deficiency, active liver disease/known risk factors for liver disease, lactic acidosis 

Interaction:

Drugs 
Zidovudine: Competitive inhibition of the intracellular phosphorylation of stavudine, concomitant use not recommended
