Indications & Dose:

Malaria, amoebic liver abscess SLE, rheumatoid arthritis. 


 DOSE



Adults


Partially immune subjects: 10mg/kg body-wt. as a single dose.


Non-immune subjects: 10mg/kg bodywt. Initially followed by 5mg/kg body wt. daily for 2 days. 


Children


 10mg/kg body wt.

Contraindications:

Presence of retinal or visual field changes, known hypersensi tivity, long term therapy in children (exception should be considered as in acute malarial attack caused by strians of plasmodia). Concurrent administration of other anti-malarials.


Pregnancy, breast-feeding: Contraindicated.

Side Effects:

Headache, confusion, convulsion, impaired hearing. hypotension, hemolysis in G-6-PD. Nausea, vomiting, epigastric distress, diarrhea, peripheral neuropathy, hepatitis, agranulocytosis, skin rash, pigmentation of nail and skin on frequent use and visual disturbances.

Cautions:

Should be used with caution in patients with hepatic disease or alcoholism, children & infants are extremely sensitive to adverse effect from an overdose of parenteral chloroquine HCl. Use with caution in severe G.I. CNS or blood disorder. 

Precautions:

During prolonged therapy periodic blood counts, liver function test and ophthalmic examination should be done

Interaction:

Antacids: reduce absorption.


Antiepileptics: antagonism to anticonvulsant effect.


Mefloquin,halofantrin:  increased risk of convulsion and arrhythmia.


Digoxin: increased plasma concentration of digoxin.


Neostigmine: incrase the symptoms of myastheniagravis.


Cimitidine: Inhibits metabolism.


Chloroquine: Idiosyncratic, drug induced involuntary movements.


Other antimalarial: Agranulocytosis.
