CEFPIROME

Indications & Dose:

Infections caused by cefpiromesensitive organisms: Infections of the lower respiratory tract. Complicated upper and lower urinary tract infections. Skin and soft tissue infections, infections in neutropenic and immunocompromised patients, septicaemia, severe infections in intensive care patients. 

DOSE

Adult: 1-2gm every 12hours by iv bolus over 3-5 minutes, or by iv infusion over 20-30 minutes Patients with renal insufficiency: Reduced in patients with impaired renal function.

Children : Not recommended

Contraindications:

Hyper-sensitivity to cephalosporins, pregnancy, lactation, cross-sensitivity to penicillin. Monitoring of renal function in case of concomitant administration of loop-diuretics or aminoglycosides.

Side Effects:

Hypersensitivity reactions: Allergic skin reactions (rash, urticaria), itching, drug fever and severe acute allergic (anaphilactic) reactions may occur. Effects on the gastrointestinal tract: Nausea and vomiting, abdominal pain, diarrhea; in very rare cases pseudomembranous colitis. Effects on liver function: Raise in serum liver enzymes (ASAT, ALAT, LDH, gamma-GT, alkaline phos-phatase), increase in serum bilirubin. Changes in blood constituents: Thrombocytopenia. Eosinophilia. Haemolyticanaemia. Granulocytopenia. Agranulocytosis. Inflammatory irritation and pain at the site of injection. Superinfection. Disorders of taste.

Drugs

Potentially nephrotoxic drugs (e.g. aminoglycosides). The Coomb’s test may give a falose-positive result. Non-enzymatic methods for the determination of serum creatinine may give false results.
