ASPIRIN /SALICYLATES

Indications & Dose:

Mild to moderate pain, pyrexia, as antiplatelet. Adult 300-900mg every 4-6 hour when necessary; max. 4 g daily after food.

Children: Juvenile arthritis up to 60-90mg/kg daily in 5-6 divided doses after food, increased in acute exacerbations to 130mg/kg.

NOTE:High doses are very rarely required and are now given under specialist supervision only and with plasma monitoring (especially in CHILDREN). 

Contraindications:

Children

Under 12 years and in breast-feeding (Reye’s syndrome, gastro-intestinal ulceration, haemophilia; not for treatment of gout. HYPERSENSITIVITY. Aspirin and other NSAID are contraindicated  in patients with a history of hypersensitivity to aspirin or any other NSAID-which includes those in whom attacks of asthma, angioedema, uticaria or  rhinitis have been  precipitated by aspirin or any other NSAID. Reye’s Syndrome. Owing to an association with Reye’s syndrome aspirin-containing preparations should no longer be given to CHILDREN under the age of 12 years, unless specifically indicated, e.g. for juvenile arthritis (Still’s disease). It is important to advice  families that aspirin is not suitable for child-ren with minor illness. 

Side Effects:

Generally mild and infrequent but high incidence of gastro-intestinal irritation with slight asymptomatic  blood  loss, increased bleeding time, bronchospasm and skin reactions in hypersensitive patients. 

Cautions:

Asthma, allergic disease, impaired renal or hepatic function (avoid if severe), dehydration, pregnancy; G6PD-deficiency.

Drugs

Heparin and oral anticoagulant: Cause haemorrhage. Antacid: reduced rate of aspirin absorption. Probenacid, Sulphinpyrazone: Decreased urate phenytion, neproxen, suphinpyrazone excretion. Thiopental, thyroxine : Increased plasma concentration leading to prolonged half-lives, therapeutic effects & toxicity.
