TELMISARTAN

Indications & Dose:


Hypertension:


Adult and Child >16 yr: 20 -40mg one daily, increased if necessary after atleast 4 weeks to max 80mg P0 one daily.


Prevention of cardiovascular events in patients with established atherosclerotic cardiovascu-lar disease, or type 2 diabetes mellitus with tar-get-organ damage



Adult: 80 mg once daily


 

Contraindications:

Side Effects:

CNS: Headache, dizziness, insomnia, somnolence, migraine, vertigo, aresthesia, muscle contractions, and hypoesthesia (0.3%-1%)


CV: Hypertension, chest pain, peripheral edema, palpitation, dependent edema, angina pectoris,tachycardia, leg edema, abnormal electrocardiogram, anxiety, depression, and nervousness (0.3%-1%)


EENT: Conjunctivitis


GI: Diarrhea (3%), dyspepsia, abdominal pain, nausea, flatulence, constipation, gastritis, vomiting. dry mouth, hemorrhoids, gastroenteritis, enteritis, gastroesophageal reflux, and toothache


GU: Impotence, urinary tract infection, increased frequency of urination, and cystitis (0.3%-1%)


MS: Back pain (3%), myalgia, arthritis, arthralgia, and leg cramps (0.3%- 1%)


RESP: Angioedema (1 case in premarketing trials), upper respiratory tract infection (7%), sinusitis (7%), cough (1.6%), pharyngitis (1%), influenza- like symptoms, asthma, bronchitis, rhinitis, dyspnea, and epistaxis (0.3%-1%)


SKIN: Increased sweating, dermatitis, rash, eczema, and pruritus (0.3%-l%)


MISC: Angioedema


 

Cautions:

Precautions:

Angioedema (associated with aspirin and/or penicillin allergy), aortic or mitral valve stenosis, biliary cirrhosis or biliary obstruction, breast feeding period, coronary artery disease, elderly patients, hepatic dysfunction (adjust dose), hypertrophic cardimyopathy, hypotension (sodium-or volume-depleted patients), pregnancy, renal artery stenosis, solitary kidney, or congestive heart failure


PREGNANCY AND LACTATION:Pregnancy category C.


HEPATIC IMPAIRMENT 20–40mg once daily in mild or moderate impairment. Avoid in severe impair-ment or biliary obstruction.


RENAL IMPAIRMENT Manufacturer advises initial dose of 20 mg once daily in severe impairment

 Digoxin: 49% increase in digoxin peak, 20% increase in trough digoxin concentrations
