Indications & Dose:

Prophylaxis of deep-vein thrombosis (general surgery)BY SUBCUTANEOUS INJECTION Adult: 3500 units for 1 dose, dose to be given 2 hours before surgery, then 3500 units every 24 hours Prophylaxis  of  deep-vein  thrombosis(orthopaedic surgery)Adult: Initially 50 units/kg for 1 dose, dose to be BY SUBCUTANEOUS INJECTION given 2 hours before surgery, then 50 units/kg every 24 hours, alternatively initially 4500 units for dose, dose to be given 12 hours before surgery, then 4500 units every 24 hours Prevention of clotting in extracorporeal circuits Extended treatment of venous thromboembolism in patients with solid tumours Prophylaxis of venous thromboembolism in patients with solid tumours BY SUBCUTANEOUS INJECTION Adult: 175 units/kg once daily for up to 6 months Treatment of deep-vein thrombosis | Treatment of pulmonary embolis BY SUBCUTANEOUS INJECTION Adult: 175 units/kg once daily until adequate oral anticoagulation established, treatment regimens do not require anticoagulation monitoring Treatment of venous thromboembolism in pregnancy BY SUBCUTANEOUS INJECTION Adult: 175 units/kg once daily, dose based on early pregnancy body-weight, treatment regimens do not require anticoagulation monitoring


 

Contraindications:

Active major bleeding; history of heparininduced thrombocytopenia (HIT);hypersensitivity to sulfites, benzyl alcohol or pork products; 1M administration


 

Side Effects:

CNS: DizzinessCV: Hypotension, hypertension, tachycardiaGI: Increased ALT,AST GU: Priapism HEME: Hemorrhage, thrombocytopenia, anemia SKIN: Local irritation, pain, hematoma and ecchymosis at injection site; rashMISC: Insomnia, confusion

Precautions:

Bacterial endocarditis; bleeding disorders; active ulcerative and angiodysplastic 01 disease; hemorrhagic stroke; recent brain, spinal or ophthalmological surgery; renal function impairment;children; neuraxial anesthesia; uncontrolled hypertension; diabetic retinopathy


 

Interaction:

Drugs Antiplatelet agents (aspirin, ticlopidine, clopidogrel,dipyridamole, NSAIDs), thrombolytic: Increased risk of hemorrhageOral anticoagulants: Additive anticoagulant effects

Pregnancy category B; low-molecular- weight heparins have been used to prevent and treat thromboembolic disease during pregnancy in lieu of . warfarin which is a known teratogen; excretion into breast milk unknown but thought to be minimal based onpharmacokinetic parameters, use caution in nursing mothers
