Indications & Dose:
Chemotherapy and radiotherapy:
Highly emetogenic regimens:
Adults: Single dose of 8mg by slow i.v inj immediately prior to chemotherapy; followed by either two doses of 8mg by i.v inj repeated 2-4 hours apart or by 1mg per hour by i.v infusion for up to 24 hours; or single dose of 32mg in 50-100ml saline infused over 15 minutes prior to chemotherapy. After first 24 hrs. followed by 8mg orally twice daily for up to 5 days. Efficacy may be enhanced by addition of dexamethasone 20mg i.v prior to chemotherapy.
Less emetogenic regimens or radiotherapy:
Adults: 8mg by slow i.v inj immediately prior to chemotherapy, or orally 1-2 hours before, followed by 8mg orally twelve hourly. After first 24 hrs, followed by 8mg orally twice daily for up to 5 days.
Postoperative prophylaxsis:
Adults: 8mg orally 1 hour prior to anaesthesia followed by two doses of 8mg orally at 8 hourly intervals; or single dose of 4mg by slow i.v or i.m inj. at induction of anaesthesia. Treatment, single dose of 4mg by slow i.v or i.m inj.
Chemotherapy and radiotherapy
Children: only: 5mg per square meter i.v immediately prior to procedure, followed by 4mg orally 12 hours later. After first 24 hrs, followed by 4mg orally twice daily for up to 5 days.

CNS: Headache, lightheadedness, seizures
CV: Angina, bradycardia, syncope, tachycardia
EENT: Blurred vision
GI: Constipation, diarrhea, transient elevation in liver enzymes
METAB: Hypokalemia
RESP: Bronchospasm
SKIN: Rash
